Case Management 
The Case Management department at Cibola General Hospital assists patients and their families with many different aspects of care, beginning when a patient is admitted to the hospital and continuing through discharge and beyond. The department provides several primary services:
Case Management
From the day a patient enters Cibola General Hospital until the day that he or she is discharged, a Case Management specialist works to ensure the best possible care is received and the best possible outcome is achieved.
Discharge Planning
This process begins the day a patient enters the hospital and doesn’t end until the patient is back at home and comfortably settled in. Discharge planners arrange for everything from durable medical equipment and in-home care, to physical rehab, home oxygen, Meals on Wheels and specialized nursing services.
Social Services
The hospital’s social worker assesses patient needs, addresses mental health and drug and alcohol dependency issues, and when needed puts patients and their families in touch with the appropriate community resources. Social Services also makes sure that patients who’ve been discharged receive adequate support at home. |
