Health Information Management 
The Cibola General Hospital Health Information Management department maintains a confidential record of diagnosis, treatment and progress for each patient. This information details the chronology of your care and gives physicians and other medical specialists the background they may need to ensure the best possible outcome.
You have the right to review and obtain copies of your health records. Requests for these records are handled in accordance with New Mexico’s laws and the policies and procedures of Cibola General Hospital. The Health Information Managment Department is open Monday through Friday, 8 a.m. to 4:30 p.m. Phone (505) 287-5249.
NOTE: Patient records are kept in the strictest confidence and in accordance with our Medical Staff bylaws, rules, and regulations, as well as all applicable state and federal laws and HIPAA (Health Insurance Portability and Accountability Act) regulations. |
