Registration
There are two Registration Offices, one located beside the Emergency Room and one near the Outpatient Entrance. Registration is open 24 hours a day, seven days a week.
When your register, the admitting clerks will ask for information that is required by law, private insurance companies, and/or Medicare and Medicaid. All patient information is held in the strictest confidence. When registering, you are asked to bring any and all of the following documents that are applicable to you:
- Active medical insurance cards
- Photo identification
- Tribal census card
- Written orders from your physician
- Social Security number
The specifics of Emergency Room registration may vary depending on circumstances and a patient’s condition, but all patients are required to complete the necessary paperwork before leaving the hospital.
It is the goal of Cibola General Hospital to make the registration process as convenient and as speedy as possible, though it should be understood that patients who need to be seen in the Emergency Room do have priority over patients registering for routine tests and procedures.
Now Offering ONLINE Pre-registration
The hospital is proud to offer patients the new option of providing pre-registration information at a time that is convenient for them, from the comfort of their own home. In addition to scheduled services, online pre-registration is also available for laboratory and other non-schedule services.
Patients should pre-register online at least 24-48 hours prior to the time they plan on coming in for services. Once the patient clicks the "Pre-register" link on the button below, all they need to do is enter the required information. This pre-registration form is on a secure website. Our goal for this pre-registration option is to reduce the amount of time a patient spends in the registration department.
On the day of the patient’s visit, staff will ask the patient to – present a photo ID; present insurance card(s); physician orders; sign treatment agreement form(s); and verify information.
Keep in mind that on the day of service patients will still need to be prepared to pay co-pays and deductibles on the estimated charges for care.

Registration Locations
Outpatient DX Registration:
When entering the hospital through the Outpatient Entrance, it is located to the left of the door. When entering the hospital through the Front Main Entrance, it is located at the end of the main hall. When entering the hospital through the Emergency Room door, it is at the end of the hall and to the right.
Emergency Registration and Inpatient Admission:
This location is off the hall connecting the Emergency Entrance and the Outpatient Services entrance. Registration office is located immediately outside the Emergency Room.
Patient Financial Services Office:
This office is located down the hall and to the right from the Front Main Entrance to the hospital. The office is located directly across the hall from the Medical Records department and down the hall from the Registration/Admissions Office.
Open Admissions Policy
It is the policy of Cibola General Hospital to admit and treat all persons without regard to race, color, gender, handicap, national origin, or religious creed. Admission requirements and assignment of hospital facilities are the same for all persons. There are no distinctions in eligibility for receiving any patient care services. Hospital facilities are available to all persons and visitors. Individuals and organizations having occasion to refer patients for admission or recommend Cibola General Hospital are advised to do so within the hospital’s policy to provide quality healthcare to all persons.
